Hydroxyapatite toothpaste rivals fluoride in early studies, but cost and long-term data lag behind

Newer mineral-based products are gaining traction due to social media buzz and health skepticism, yet dental experts still cite fluoride as the standard of care due to decades of extensive research.

Dentists across the United States are fielding a growing number of inquiries about fluoride-free toothpaste, a shift driven by social media promotion and persistent public hesitation regarding the mineral’s safety. Dr. Jin-Ho Phark, an associate professor of clinical dentistry at the University of Southern California, notes that patients frequently ask about calcium phosphate minerals, specifically hydroxyapatite. He attributes this surge to influencers aggressively marketing these alternatives on digital platforms.

The traditional fluoride toothpaste became the national standard in the late 1950s when Procter & Gamble released Crest. The Centers for Disease Control and Prevention identifies the addition of fluoride to public drinking water as one of the top ten public health achievements of the 20th century, noting that it reduces cavities by 25 percent. However, recent criticism from Health Secretary Robert F. Kennedy Jr., who has linked fluoridation to conditions ranging from arthritis to neurodevelopmental disorders, has fueled consumer interest in mineral-based alternatives.

Hydroxyapatite mimics the composition of tooth enamel, which is primarily made of calcium and phosphate. Dr. Livia Tenuta, a dentist at the University of Michigan School of Dentistry, explains that fluoride protects teeth by remineralizing them. It combines with minerals in saliva to form a protective shield on the enamel. Hydroxyapatite operates differently, with nano-sized particles designed to penetrate tiny crevices in the tooth surface to help repair sensitivity and decay.

Research comparing the two ingredients is emerging, though it remains limited. A 2021 study in Scientific Reports tracked 177 children for nearly a year and found similar outcomes between hydroxyapatite and fluoride toothpaste. That study received funding from Dr. Wolff Group, a German company that owns brands producing hydroxyapatite toothpaste. A follow-up study published in Frontiers in Public Health in 2023 tracked 171 healthy young adults for 18 months and also found comparable results in preventing tooth decay.

Dr. Bennett Amaechi, a professor of dentistry at the University of Texas at San Antonio and an author on the 2021 study, states that hydroxyapatite appears as effective as fluoride for remineralization and preventing caries. However, he emphasizes that fluoride possesses a much more extensive body of long-term research. For patients open to both options, Amaechi advises using fluoride due to the stronger evidence base supporting its efficacy.

The American Dental Association acknowledges the rising popularity of hydroxyapatite products. Dr. Ada Cooper, a spokesperson for ADA Consumer Advisor, wrote that while some evidence suggests the mineral contributes to remineralization, the clinical data remains limited and ambiguous regarding specific indications. The organization continues to recommend brushing twice daily for two minutes with fluoride toothpaste, flossing daily, and visiting a dentist regularly.

Consumer prices for hydroxyapatite toothpaste often exceed those of standard fluoride brands by more than a dollar per ounce. Dentists like Tenuta note that most existing studies involved participants with low risks of tooth decay. She indicated a need for more long-term studies involving patients with higher caries risk to fully validate the effectiveness of the mineral-based alternative in broader populations.
